Medication Log Book for Carers

This medication log book is designed specifically for carers, both in nursing/care home environments as well as home visiting carers.

The book will log up to 11 patients, with an easy to complete contents page for you to fill in the details for each patient in your care. The printed tab nature of this contents page corresponds with the relevant patient page within the book for easy reference as well as printed number pages – also referencing the correct page number of that particular patient. This is to clearly define ‘patient sections’ within the book and to prevent getting any patients mixed up. There is also a solid printed black divider page between patient sections to further segregate log book pages.

Each Patient section allows you to log medical particulars (blood group/allergies/height/weight etc) as well as emergency contact numbers (relatives and doctors) for that particular patient. Then follows eight medicine log pages; each of these is designed in a ‘medicine per page’ format, so you simply fill in date and time for each. This is rather than ‘day per page’ format which would take more time as you would have to repeatedly fill in the medicines and dosage each time.
